If you want to use a Response Form again, you should copy or Clone the Response Form. This way, you can make changes but keep whatever elements of the form you would like to remain the same. Even if you have no changes to make to the form, if you're using the form again at a different time, it's a good idea to copy/Clone the form so that the new responses are separate from the old responses.

Just click the Clone link that appears in the Action column on the Manage Response Forms page.
When you clone, the original form is preserved and can continue to be linked to and used. The newly cloned form has the same name as the original form, so be sure and Edit the new version right away and change the form name. You’ll be given a new url to use in your creative for the new form.
